The engines are constantly pushed to work in extreme circumstances and pushed to be a "useful engine". Non-useful engines are dismantled and become chicken coops (RIP Smudger).

The Fat Controller is also completely A-OK with live burials, as Henry gets walled in to spend the rest of eternity inside a damp cave. OHSA is also non-existent in the island of Sodor, you have engines falling off cliffs and running into ditches, but its all ok because "no one was hurt", not counting the trains themselves obviously.

Institutional racism is also rampant thoughout the island. All the Diesel engines are sent to work in the quarry and the mines, where they all suffer from abestos poisoning, while the steam engines get cleaned and loved by all.
